Policies and Procedures

Below is an example of what you will review and sign in our office at your visit.

Thank you for choosing Integrated Pathways and entrusting us with your child’s development and therapy needs.

Please read thoroughly and initial beside each statement. If you have questions about the policies listed below, please ask your therapist for clarification. 

▢ I understand that Integrated Pathways will charge $25 for 60-minute sessions and $40 for 90-minute sessions to families (non-Medicaid) who do not call, text, or email at least 4-6 hours in advance to cancel, or who do not attend their appointment. 

▢ I understand that if I ‘no show’ at least three appointments, my child will be discharged from Integrated Pathways.

▢ I understand that if I repeatedly cancel appointments (30% of scheduled visits within 12 weeks), I may be discharged from the practice or placed back on the waitlist until a more suitable appointment becomes available. 

▢ I understand that I must review my insurance policy and benefits. This includes understanding how many OT visits I am allotted, my deductible, and any copays that will occur after I meet my deductible. I understand that it is not the responsibility of Integrated Pathways to provide this information. Integrated Pathways will provide your benefit information upon request from Stephanie Cooper. stephanie.cooper@medcredips.com

▢ I may be responsible for a portion of my copay at the beginning of my session until my deductible is met. 

▢ I understand that my balance with Integrated Pathways should not exceed $250, or my child will be placed on hold until the balance is paid. 

I understand that Integrated Pathways cannot receive replies to reminder texts or emails. If I need to cancel my appointment, I must contact my therapist directly or text 423-645-2746
